Job Objective :
This position works under the direction of the Quality Data Program Manager or designee; the QM Data Specialist will support the needs and operations of the Quality Data Program.
The Quality Data Specialist is responsible for the abstraction of clinical data from a patient’s medical record and input into an electronic database(s) that will be used for analysis, process improvement, and public reporting.
Duties and Responsibilities: Review of medical records and other source documents to collect clinical data of patients in the measure according to the specifications and guidelines of the applicable data element and measure specifications.
Uses various software applications as needed.
Abstracts information in a timely manner within established timeframes.
Keeps abreast of regulatory/accreditation/health quality partner changes.
Conducts searches on the internet for regulatory and accreditation and quality partner site information.
Responsible to remain up to date with data definitions as related to abstraction of the data.
Educates oneself using various methods provided such as web-based sessions, regularly scheduled abstraction clinics & seminars, and frequently asked questions forums.
Under the direction of the Quality Manager/Data Manager, will participate in tasks to ensure inter rater reliability and validity of abstraction.
Abstraction accuracy shall achieve as close to 95% as possible but not lower than 85% of quality measures assigned for more than 1 quarter.
Notifies Director whenever time frames cannot be met.
Works collaboratively with team members as required during review of the medical record.
Assists Data Analyst/Quality Manager with audits & survey preparations.
Assists Data Analyst with performance improvement or special projects as needed.
Refrains from disclosing confidential information acquired in the course of work except when authorized and unless legally authorized to do so.
Performs additional Quality studies and duties as directed by Quality Manager/Data Analyst.
Follows Administrative policies on Confidentiality & Privacy Practices.
Non-Essential Duties: Other miscellaneous duties and responsibilities as required for maintaining the services provided by the department.
Knowledge, Skills and Abilities: Knowledge Medical terminology knowledge strongly preferred.
Skills Able to communicate effectively in English, both verbally and in writing.
Comprehensive knowledge of medical diagnostic & procedural terminology.
Understanding of disease processes, anatomy & physiology as necessary.
Skilled in data analysis and interpretation.
Knowledge of federal and state regulations and requirements which pertain to patient care information.
Computer skills to include data entry and intermediate knowledge of MS Office Professional (Outlook, Access, Excel, PowerPoint and Word).
Abilities Critical thinking and problem-solving abilities.
Ability to organize information, people and resources.
Ability to function independently with minimal supervision.
Ability to work collaboratively with and through others.
Ability to multitask, work independently & organize workload in a timely manner.
Ability to interpret criteria for review and abstract data element requirements as defined.
Ability to read handwritten and transcribed documents in the medical record, interpret information, and enter complete and accurate data into a computer system.
Ability to use designated reference materials.
Education and Experience: Education Associate’s degree in health-related field or two years’ experience in health-related field.
Experience 2 years of demonstrated experience in data abstraction and/or medical coding.
Medical Terminology.
Required Licensure and/or Certifications: Certificate in Health-related field preferred.

Must be able to make rounds within hospital and conduct patient/family interviews as well as conduct education throughout hospital as needed.
Must be able to use standard office equipment, sit, and perform occasional bending and twisting.
Ability to lift and carry 10 lbs.
Potential exposure to biohazards-bloodborne infectious waste and ability to use PPE0.
