What will you be doing:
The Nurse Practitioner (NP) is responsible and accountable for the application of the nursing process, supervision of clinical practice, research involvement, consultative services, inter and intradepartmental liaison, education, leadership, and patient care management across the health care continuum from preventative, acute care through discharge.
Nurse Practitioner’s practice incorporates both independent decision-making abilities and collaboration with the board certified/eligible physician to provide expert care to the patients.
As a Nurse Practitioner, you will be responsible for evaluating the patients (includes chart review, performing history and physical examinations, ordering diagnostic tests, reviewing results and reporting to the Physician as needed.
Provide services to Hepato-pancreatico-biliary (HPB) and Liver Transplant patients, timely documentation of patient progress notes, after visit summaries, and CMS required care coordination notes.
Coordinate communication of patient ADT and surgeries with referring and collaborating MDs.
Develop and implement patient management plans and assist in provision of continuity of care.
Provide education, leadership, and patient care management across the health care continuum from preventative and acute care.
The NP serves as a patient advocate throughout the care process and represents the medical center at regional and national meetings/conferences.
Provide Inpatient services to Hepato-pancreatic-biliary (HPB) patients, coordinate daily activities of inpatient service, including facilitation of early discharge and care coordination/patient education, timely documentation of patient progress notes, discharge summaries, and CMS required care coordination notes.
Coordinate communication of patient ADT and surgeries with referring and collaborating MDs.
Develop and implement patient management plans and assist in provision of continuity of care.
Provide education, leadership, and patient care management across the health care continuum from preventative and acute care through discharge.
Patient advocate throughout the care process and represents the medical center at regional and national meetings/conferences.
There is a possibility of rotation in inpatient and outpatient services.
You will provide direct patient care on a rotating 10-hour shift schedule that includes weekdays, nights, weekends, and holidays.
Experience Requirements:
Two (2) plus years of acute care hospital nursing experience.
Nurse Practitioner experience preferred.
Educational/Licensure/Certifications Requirements:
Master of Nursing Degree
Graduate of an accredited Nurse practitioner program approved by the California Board of Registered
Current State of California Registered Nurse and Nurse Practitioner License
National Certification or eligibility for certification as a Nurse Practitioner (ANCC/AANP)
Current DEA license and Furnishing license or eligibility to acquire
Current BLS and/or ACLS certification required
